TEXAS CITY INDEPENDENT SCHOOL DISTRICT ATHLETIC DEPARTMENT

Dear Parents of TCISD Athletes:

To prepare our students for the upcoming athletic seasons, there are several required forms and procedures that need your attention. The following items will ALL be required to clear your child for participation and must be completed PRIOR to participation in any try-out, workout, practice, or game (in-season or off-season):

1. UIL Physical & Medical History Forms
a. Physicals must be dated after April 1 st to be accepted for the upcoming school year. All physicals expire at the end of each school year, no matter the date of the exam.
b. Forms may be printed by selecting the “DOWNLOAD AND PRINT” tab. Select the Medical Information Form Cover Page link and Physical & Medical History Form link. We encourage you to fill out the medical history prior to printing the form.

2. Athletic Participation Forms
a. This form will be electronically submitted at any time after April 1 st . Once this section has been submitted and accepted, you will receive a confirmation email for your records. You must enter an email address.
b. Parent/guardian must have student present along with student ID to complete the form.

You will be asked to provide information such as:
  • Student ID
  • Phone numbers
  • Email addresses
  • Name of primary care physician
  • Insurance policy information, and other pertinent information


Please make sure you have this with you when filling out your forms.

Remember that to submit online appropriately, you AND your child must fill out and sign all required spaces. Forms will not submit electronically without all required information being completed.

To sign the document, click inside the signature box and hold your mouse down, this will allow you to create and “Electronic Signature.” If you make a mistake and need to start over, click on the refresh icon next to the signature box. If you are using a tablet, you may create an electronic signature by holding your finger to the screen and signing your name or by using a stylus.

How to scan documents on your iPhone or iPad:

  1. Open Notes and select a note or create a new one.
  2. Tap the Camera button, then tap Scan Documents.
  3. Place your document in view of the camera.
  4. If your device is in Auto mode, your document will automatically scan.
  5. Tap Save or add additional scans to the document.


How to scan documents on your Android:
  1. Open the Google Drive app.
  2. At the bottom right, tap Camera.
  3. Point your device’s camera at the document.
  4. Take a photo of the document that you’d like to scan.
  5. Adjust your scanned document.
  6. Tap Done.
  7. Create your own file name or select a suggested title.


FAQs

I received an email stating my athlete’s physical was denied. Why?
Medical History and Physical may be denied for a few reasons: The upload does not include an attachment. An icon will blink yellow upon attaching your file, wait for it to turn green before submitting. Form(s) are blank (medical history must be completed before seeing a physician; physical examination form must be completed by a physician) Date on the form(s) is no longer valid Physical states that the athlete did not pass the physical (If you have supporting documentation to accompany a failed physical, please contact your athletic trainer directly) Form is incomplete (missing identifying information, signatures, and other pertinent details) The incorrect document was uploaded (please do not upload immunization records, screenshots of patient portals, or any other variations of medical history and/or physical forms); the physician must use the UIL pre-participation physical form

I can’t find my athlete’s account in RankOne. What should I do?
Please make sure there are no typos in your athlete’s student ID number or last name. Athletes with multiple last names should consider including both names, hyphens, or any other variances before contacting the campus athletic trainer. Athletes in 6th grade will not have an account in RankOne until after May 1st of that school year. Please wait until May 1st before contacting the campus athletic trainer. Athletes new to the district may need to have an account created within RankOne. Your campus athletic trainer will be able to assist with this process.

My athlete’s account is linked to the wrong school. What should we do about this?
The school will be corrected over the summer. This inaccuracy will not affect your ability to upload and complete online forms.

I have uploaded and completed my student’s medical history, physical, and electronic forms, but it still shows pending approval.
Physical uploads are reviewed by your campus athletic trainer in real-time. Please allow at least 24 hours of processing time during normal school year operation.

I keep trying to upload my documents, but they are not attaching.
Faulty uploads should trigger a pop-up that states “wrong extension”. Your file was not successfully uploaded, and you need to try again.
