TEXAS CITY INDEPENDENT SCHOOL DISTRICT ATHLETIC DEPARTMENT
Dear Parents of TCISD Athletes:
To prepare our students for the upcoming athletic seasons, there are several required
forms and procedures that need your attention. The following items will ALL be required
to clear your child for participation and must be completed PRIOR to participation in any
try-out, workout, practice, or game (in-season or off-season):
1. UIL Physical & Medical History Forms
a. Physicals must be dated after April 1 st to be accepted for the upcoming
school year. All physicals expire at the end of each school year, no matter
the date of the exam.
b. Forms may be printed by selecting the “DOWNLOAD AND PRINT” tab.
Select the Medical Information Form Cover Page link and Physical &
Medical History Form link. We encourage you to fill out the medical history
prior to printing the form.
2. Athletic Participation Forms
a. This form will be electronically submitted at any time after April 1 st . Once
this section has been submitted and accepted, you will receive a
confirmation email for your records. You must enter an email address.
b. Parent/guardian must have student present along with student ID to
complete the form.
You will be asked to provide information such as:
- Student ID
- Phone numbers
- Email addresses
- Name of primary care physician
- Insurance policy information, and other pertinent information
Please make sure you have this with you when filling out your forms.
Remember that to submit online appropriately, you AND your child must fill out and sign
all required spaces. Forms will not submit electronically without all required information
being completed.
To sign the document, click inside the signature box and hold your mouse down, this will
allow you to create and “Electronic Signature.” If you make a mistake and need to start
over, click on the refresh icon next to the signature box.
If you are using a tablet, you may create an electronic signature by holding your finger
to the screen and signing your name or by using a stylus.
How to scan documents on your iPhone or iPad:
- Open Notes and select a note or create a new one.
- Tap the Camera button, then tap Scan Documents.
- Place your document in view of the camera.
- If your device is in Auto mode, your document will automatically scan.
- Tap Save or add additional scans to the document.
How to scan documents on your Android:
- Open the Google Drive app.
- At the bottom right, tap Camera.
- Point your device’s camera at the document.
- Take a photo of the document that you’d like to scan.
- Adjust your scanned document.
- Tap Done.
- Create your own file name or select a suggested title.
